Continuing Education Information
Certificate of Completion & Credit Reporting
In order to receive the certificate of completion and earned credits reported, individuals must complete the Evaluation and either return it to a Foundation staff member in-person at the conference or send the Evaluation to Bronwen Kearse (bkearse@crohnscolitisfoundation.org) via email. Once the Evaluation has been received, then a certificate of completion will be provided to the individual and their earned credits will be reported.
One completed Evaluation is required for the full day.
Accreditation Statement
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Medical Association of Georgia through the joint providership of Southern Alliance for Physician Specialties CME and Crohn’s and Colitis Foundation. The Southern Alliance for Physician Specialties CME is accredited by the Medical Association of Georgia to provide continuing medical education for physicians.
Credit Designation Statement
The Southern Alliance for Physician Specialties CME designates this live activity for a maximum of 3.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Other Healthcare Professionals
Other healthcare professionals who participate in this CE activity may submit their statement of participation to their appropriate accrediting organizations or state boards for consideration of credit. The participant is responsible for determining whether this activity meets the requirements for acceptable continuing education.
